Enrollment Process
To expedite the enrollment process, please make sure you turn in the following information (also listed on the enrollment packet), as well as any additional documents. For questions, please contact the office.
Age Verification
Certified Birth Record, Duly Attested Baptism Certificate, Passport, County Registrar or Recorder Statement, Affidavit by the Parent/Guardian or Other
Proof of Required Immunizations
Official copy from Doctor, or previous school.

statement, contract or payment receipts in the parent/guardian's name.
Other documents accepted:
Property tax payment receipts
Rental property contact, lease, or payment receipts
Pay stubs
Voter registration
Correspondence from a government agency
If parent/guardian is unable to provide any of the documents listed as proof of residence, please contact your school.
Educational Documents (If Applicable)
Please bring latest copy of your Individualized Education Plan (IEP) or copy of 504 Accommodation Plan if applicable.
Court Documents (when applicable):
Legal Guardian, Foster Parent, Custody Orders, Restraining Orders.
Other Documents
